Case 1 is dependent on those values... if you have a 10 attack and a 17 defense you have a 50/50 chance, but if you have a 10 attack vs a 18 defense your chance decrease, with a 9 more so, and so on... similarly if you have a 12 attack vs a 16 defense you're chances of hitting are something like 80%
Case 2 - did you hit? Can they hit you back?
Case 3 - same question, though SS are always nice to have as it gives you an extra change to avoid being hit no matter what case 1 was

Well this must be a trick question, but I like it.
The answer, of course, is it's better to have both. In your examples, it seems you are referring to individual characters, but the question has to do with building a force, which implies making up a team. And when you are talking about a team, you must have both offense and defense to be successful.
If we are talking about individual characters, then the question becomes what role does this character play on your team?
First, let's talk about the prototypical team build:
A. Brick (Long dial, damage reducers, SS + mobility a plus) B. Ranged support (6+ range, good damage, good mobility, One of the Big 3) C. Range Specialist (8+ range, good attack, high damage, minimal defense) D. Close combat harasser (Decent damage, good attack, good defense, good mobility)
Now this isn't the only way to build a team, but most teams are a variation of this build. Certainly most winning teams are. So when I am looking at characters, the question I ask is what role will they be playing? For A, the emphasis is on defense. They should be able to soak up damage in one way or another, usually absorbing it via damage reducers. For C, attack and damage are the priority. Defense is optional since the plan is to have A and D run interference while death is dealt from a distance.
So if I had to choose between offense or defense in a philosophical sense, which would I pick? I would pick defense. No team has won with an offensive team. In fact, this is the biggest problem with the X-Men. The X-team options consist of mostly B and C, with a smattering of D (Wolverine and a few others) and very few of A. An offensive team can't survive counter attacks, much less aggressive moves by your opponents, without being extremely lucky. Over the course of 9 games, your luck will run out, gauranteed.
However, with a solid defense you can weather most storms and as long as you have decent damage you will eventually make it through. And in IQ, it's not about how your team plays when everyone is your friend, because eventually someone will be out to get you and you need to be prepared.
